Florida does not mandate a specific rental application form. Landlords may charge an application fee but must disclose the amount upfront. There is no state cap on application fees.

7. FCRA Authorization, Consent, and Adverse Action Notice

Authorization: By signing below, Applicant authorizes Landlord and its agents to obtain consumer reports, credit reports, criminal background reports, eviction reports, and to verify employment, income, rental history, and references for the purpose of evaluating this rental application.

FCRA Notice (Fair Credit Reporting Act): If any adverse action is taken based in whole or in part on information contained in a consumer report, Landlord must provide Applicant with:

1. Notice of the adverse action.

2. The name, address, and telephone number of the consumer reporting agency that provided the report.

3. A statement that the agency did not make the decision and cannot explain why the decision was made.

A copy of the report may be obtained free of charge upon request within 60 days. Under Florida Statute §83.49 and FCRA, if a prospective tenant is denied based on a consumer report, the landlord must provide the required adverse action notice.

Applicant understands that this authorization is voluntary and may be revoked in writing prior to the report being obtained.

8. Fair Housing and Nondiscrimination Notice

Landlord complies with the Federal Fair Housing Act, Florida Fair Housing Act, and all applicable local fair housing laws. We do not discriminate on the basis of race, color, religion, sex, national origin, familial status, disability, or any other protected class.

9. Source of Income

Landlord accepts all lawful sources of income for rent payment, including but not limited to wages, alimony, child support, Social Security, disability, and housing assistance programs, in accordance with applicable law.
